The undersigned agrees to save harmless, indemnify and release Salsa Babies, its proprietor, employees,
heirs, assigns and any other participants from any liability, cause of action or damages howsoever caused,
including but not limited to any act, error or omission on the part of the undersigned, Salsa Babies, its
proprietor, its employees, heirs, assigns, or any other participants. Further, the undersigned authorizes
Salsa Babies to take all reasonable steps to respond to a medical or other emergency, including but not
limited to the providing of immediate first aid and obtaining professional medical assistance, and the
undersigned shall hold Salsa Babies and its representatives harmless and release Salsa Babies, its
proprietor, employees, heirs, assigns and any other participants from any liability, cause of action or
damages howsoever caused from any misfortune, malady or tragedy, physical or otherwise, that may
arise due to the participation by participant and the participant's child(ren) in the Salsa Babies session.
The undersigned further agrees to release Salsa Babies, its staff and
members from all claims for damages howsoever caused, including but not limited to any act, error or
omission, that are caused by or arise from the participation of the participant named herein during the
instructional program of Salsa Babies.
The undersigned has obtained consent from its medical practitioner and/or family doctor to partake in the Salsa Babies session.
Terms:
Registration is on a first-come, first-served basis. Advanced registration is recommended, however if you are
unsure whether the program is right for you, please contact us to arrange for a trial class.
Cancellation:
On the rare occasion that we are forced to cancel classes due to weather or other circumstances beyond our
control, we will make our best effort to contact you in a timely manner. A make-up class will be scheduled.
Missed Classes:
Each participant is entitled to one (1) make-up class per session. Classes can be made up either at another
location during the same session, or into the following session.
Refunds:
No refunds will be provided.
